Documentation & Identification

• Original birth certificate or certified copy

• Proof of residency (utility bill, lease agreement, or mortgage statement dated within 45 days)

• Florida Immunization Certificate [ Fl. Statute 1003:22]

• School Entry Physical completed with the last 12 months.

• Photo ID for parent/guardian completing enrollment

• Custody documents (if applicable)

• Previous school records or transcripts (if transferring)

Special Education & Services (if applicable)

• Previous IEP (Individualized Education Program) or 504 plan documentation

• Psychological or educational evaluation reports
